Over the years, pacing strategies have evolved from more traditional right ventricle pacing to cardiac resynchronization therapy using left ventricular pacing, especially in patients with reduced left ventricular systolic function. The detrimental effects of chronic RV pacing have been well established. More recently, permanent conduction system pacing (PCSP) utilizing either His bundle pacing (HBP) or left bundle branch area pacing (LBBAP) has emerged as an alternative to other forms of pacing due to its physiological nature of exciting the native conduction system, thereby maintaining electrical synchrony. It is still unclear if this pacing modality is feasible and will offer the same benefits in children and patients with CHD, specifically in those with complex CHD or those with repaired VSD were the implant of lead in the septum might not be feasible. Neither do we have data regarding the durability of this type of pacing in our patient population, either. With the combined efforts of PACES and AEPC, we aim to start phase I of the study and collect data on the feasibility, efficacy, safety and outcomes of PCSP in pediatric patients as well as all patients with CHD . All patients with “intention to treat” should be enrolled so we can determine not just success, but also failure to implant a conduction system pacing lead. The second phase of the study will include a comparative cohort with 1) patients with RV pacing and 2) CRT.
